Your patient, Mr Edward Teo presents to the primary health care facility and has completed his first 75+ annual health assessment. click here for a copy of the health assessment for MrTeo.On reviewing this document, you are required to map a care plan to identify the six (6) main issues that affect your patients’ independence, with the main priority issue outlined as Problem 1 in the template. An appropriate issue to be chosen is one that a) is currently affecting your patients independence, b) will affect his future independence and/or c) there is solid evidence from research regarding the risk and the impact on older adults. Evidence based practice must inform both your nursing interventions and rationale.Your ability to link concepts will be essential when completing this care plan, as your patient will present with secondary signs and symptoms, and have an established past medical history that will need to be considered and integrated into your care plan. Additionally, your care plan must reflect VIPS, as outlined in Moyle, Parker and Bramble (2014).Your mapping should consider Mr Teo's holistic wellbeing, and have a primary health care focus. For each issue identified, a referral must be included to a service that is available in your community, that will assist Mr Teos independence or wellbeing. You are required to outline the referral process for each service; an CHSP assessment MUST be included as one of your services, with specific reference to what service (one) offered by CHSP you suspect your patient may benefit from, with justification from research.You may use poetic license (that still reflects the information within the health assessment) to expand on details of your patients’ life, in order to complete a holistic care plan.
