You will write a report AS IF you were a Practice Nurse at a General Practice surgery. The report is to be written for the GP about a patient you have assessed. As the imaginary Practice Nurse, you will choose relevant assessments for the patient in the case study. The report will be structured around four questions which will be answered in 400 words (4 questions x 400 words = 1,600 words). The questions focus on - person-centred care; age related physiological changes; significance of the assessments you have chosen to undertake and strategies for optimal health and keeping the older person in their home by evaluating the suitability of service provision and referring appropriately. Your answers to the questions will be written in third person and substantiated with current sources (textbooks, government reports and journal articles).
