You are required to submit an essay to show your ability to critically analyse the care you have given to a selected client/patient who has been diagnosed or lived with diabetes: Please note the word limit. If an assessment is in excess of 4500 words your work might be penalised by the deduction of the marks. Structure Title Introduction (300 words*): The selected topic should be clearly identified by giving a clear outline of the chosen patient and a relevant background or information. Consider diabetes prevalence, quality outcome framework, State of the Nation or other primary sources to support your chosen topic. The aim of your care analysis and the issues you would like to analyse should be clearly identified. Make sure that the patient’s confidentiality and the trust information are protected (NMC, 2015). Main body (3900 words*): The patient’s biography and their biopsychosocial assessment should be clearly explained. The pathophysiology should be closely related to the patient’s condition (for example, only discuss the pathophysiology of type 2 if you have selected a patient with type 2 diabetes. Similarly, if you select a patient with diabetic foot ulcers, focus the pathophysiology on diabetic foot ulcers). You do not need to cover all aspects of their care management, but make sure that you analyse the current issues and their management relevant to your role holistically and give sufficient research-based-evidence to support your actions. When necessary, evaluate the effectiveness of the patient’s care. You also need to include the contributions from the multi-disciplinary team in the discussion. Please also remember that this essay is not submitted as a report but as a care analysis supported by current evidence.
