You are required to complete a 2000 word patient journey mapping assignment, worth 50% of your overall course grade (not including tables or reference list).You will use the Managing Two Worlds Together (MTWT) patient journey mapping tools to explore and critically analyse the access and quality of care issues that are faced by Aboriginal and Torres Strait Islander people when seeking health care. You have been given multiple case study readings in the first 6 weeks of this course that use these tools. These readings and case study examples will help guide you in writing this assignment. The mapping tools were also discussed in the face to face workshop on 2nd August.You may be able to identify a person of Aboriginal and/or Torres Strait Islander origin during one of your placements (past or present) and map and analyse their patient journey using the templates. If you are unable to identify a suitable candidate, or have not yet had clinical placement, then you can use one of the three case studies provided: Tieandra’s story, Uncle Moogy’s story, and Jemmas story.Tips to assist you with this assignment are below:Structure: 10%Give an introduction to the assignment and describe the way in which the assignment will proceed.Provide a rationale for why you chose this patientStructure the assignment logically, thereby it has an introduction, discussion and conclusion that summarises what you have discussed.The assignment is structured in a logical sequence.
