Write a Report on the Admission process i.e. Assessments taken, documentation required, discharge planning and care plan formation and implementation of one  culturally diverse client during your clinical placement that you have admitted under the supervision of a Registered Nurse or Enrolled Nurse. These patients may or may not be ready for discharge by the end of your placement. This report is to reflect your ability and understanding of the assessment process, planning and ongoing change of patient care while in hospital in light of their cultural beliefs, understanding and expectations. You must have the permission of the facility you are working in and the client’s to use this information noting that there is to be no reference to the Patient’s name, Family name or Facility name. Please include in your report the following: Admission diagnosis, Patient’s assessments and understanding on admission, relevant past history, cultural background, social supports  and their understanding, independence level, client expectations, Allied Health Team involvement and impact, discharge planning strategy, diagnostic testing and its implications, accuracy of nursing handover reflecting patient’s current condition and needs. The clients overall acceptance of and participation in care and all education given to the client and towards discharge.
