Professional Experience Placement Analysis
The professional experience placement analysis (PEPA) helps to provide knowledge and skills in the reflective and disciplined way (Gilburt, Slade, Bird, Oduola & Craig, 2013). A detailed care plan has been designed for to offer a holistic as well as patient-centred care. The study includes the nursing assessment, diagnosis, expected outcome, intervention and outcome assessment.
Patient Introduction
Mr X is a vulnerable young adult. He is diagnosed with schizophrenia. He was admitted to the psychiatric unit as he is experiencing various mental problems in recent days. He is homeless and does not have sufficient social support network. He has experienced traumatic abuse during childhood. Although he has a strong relationship with grandmother; however, he is identified to be vulnerable.
He is exploited by his family members and peers that have a negative effect on him. As a result, he is suffering from substantially reduced life expectancy. He has adverse cognitive symptoms also, which include hallucination, abnormal motor behaviour, delusions, limited emotional expressions, avolition, alogia and anhedonia. He has no record of allergy and has also a family history of schizophrenia. The Registered Nurse has given Aripiprazole 10 mg nocte, Temazepam 10 mg PRN nocte, Olanzapine 2.5 mg nocte and Diazepam 2.5 mg QID as prescribed by the general physician in the psychiatric unit.
Assessment
In the nursing assessment for Mr X, it is necessary outlining Mental Status Examination, risk assessment as well as physical health examination. American Psychiatric Association (2013) referred MSE as the systematic appraisal of the present mental status of the person suffering from mental health illness. According to Patel, Cherian, Gohil and Atkinson (2014), this is done via interaction with as well as observation of the patient. On the other hand, risk assessment helps to identify the factors, which influence the occurred potential harms. Plante et al. (2016) mentioned that in case of physical health examination the Registered Nurse uses the head-to-toe framework to collect the subjective data and objective data also.
Mental status examination
For the MSE, ASEPTIC framework is used that is Appearance and behaviour, Speech, Emotion, Mood and affect, Perception, Thought, Insight and judgement, Cognition (Norris, Clark & Shipley, 2016). The Registered Nurse helped Mr X to appear neat and maintain personal hygiene. He had maintained eye contact with the nurse and used polite words during the communication. The speech was slow and the motor response was delayed. During the conversation, he mentioned about the suicidal mentality and anxious feelings of him.
Further, he stated that since few days, he has been suffering from sleeping disorder, as he stays awake late in night and arises early in the morning. He conveyed that sometimes he thought to use substance abuse; he has poor health and social outcomes, which contribute to the significant mortality and morbidity. He has low concentration level as well as poor memory.
