The elements in clinical assessment that lead to a DSM-IV-TR diagnosis include instability in behavior and reaction as well as undue violent and aggressive or abnormal behaviors. Mental health and emotional responses form an extremely important element of being fit. Mental illnesses, even in its initial stages are diagnosed through behavior analysis, reactions and counseling. These clinical assessments may help to a great extent in diagnosing a patient with mental illnesses or a DSM-IV-TR diagnosis. This helps ensure appropriate and timely treatment (Rookes, P. & Willson, 2000). 
Discussion
Depression, Excessive anger, emotional imbalance and several such DSM-IV-TR diagnosis are possible through clinical assessment. Through the process of counseling it is possible for a psychiatrist to diagnose mental illnesses. Interventions are essential while treating a patient or a mentally disturbed person and counseling has an immense role to play in it. 
Psychosocial incorporates feeling, thinking, acting and relationship with others or people around you. It also covers the ability to adapt with the changes and cope with the stress coming in the life and how efficient is the person to develop values and beliefs. Psychosocial functioning directly and indirectly impacts behavior as well as helps in building the required strength to cope with trauma (Kraemer, H. C., Wilson, G. T., Fairburn, C. G., & Agras, W. S, 2002). 
Conclusion
Basic irrational assumption refers to assumptions which have no basis for its formation or for them  to be believed. They are untrue and an easily be refuted on the grounds of invalidity and irrationality and when this is diagnose it results in clinical assessments of a DSM-IV-TR diagnosis. It is extremely important that there is a strong classification formed where in it is easier to identify the category and possible treatments of a mental illness under all circumstances.
