Cirrel River Disability Services SUPPORT PLAN for Jean Attachments – NDIS Plan Goal: 1.       Jean will increase her independence by being able to prepare three simple meals needing only minimal supervision by December 30 Activity Resources (NDIS) Person responsible Time frame Jean participates in an assessment of her learning needs 2 hours – assessment of skills and safety, writing report  to inform teaching strategies OT Month 1 Jean builds skills in preparing: a bowl of cereal a Vegemite sandwich baked beans 4 hours getting to know Jean and understanding how to most effectively support her in positive ways     44 hours – 2 hour a day x 2 days a week for 8 weeks and 1 hour a day x 1 day a week for  a further 12 weeks – 20 weeks total   3 hours to develop teaching program   1 hour to conduct review at end of program Support worker         Support worker       Support planner   Support planner       Months  2 - 7 Jean is assisted in the food preparation by her father and sister 1 hour each day Father and sister     Possible risks Possible risk Strategy for minimising/removing risk Where is this reflected and resourced in the Support Plan? Jean’s behaviour may interfere with her ability to learn Strategies for supporting Jean’s positive behaviours are understood and implemented by the support workers.         Support workers will have 2 hours at the start of the program get to know how to implement June’s behaviour management plan so that they can support her in a positive way. Jean may have an accident in the kitchen if not fully supervised such as burning herself on the hotplates, or cutting herself with a knife. Level of risks including burns and cuts will be assessed by the OT and teaching strategies will be adapted to include how to be safe in the kitchen. Meals chosen will be ones that Jean enjoys but do not require a sharp knife to be used. If the assessment indicates that Jean may not be able to learn safety  xxx   Jean may abscond from the family home if her father and sister are not there and she is not fully supervised by a support worker. Jean will not be left alone in the family home or garden  - a schedule ensuring either family or support worker are with her at all times   Jean will not engage with the support workers due to her limited interpersonal skills and so will not participate in activities. A consistent small team of people will support Jean through the program. Decision making
It can be said that an individual must make their own decisions with regard to their life. Needless to say that there are some of the people who do not possess that capability for taking decisions in their life especially the people who have some disability (Stacey et al., 2017).
As per the support plan of Jean it has been observed that there some of people who supported her for making sound choices and decisions in the procedure.
