Using paragraph format, chronologically address the following questions in accordance to the action plan you designed. 1. What was your intrinsic and/or extrinsic motivation for choosing the activity or behavior you did? Describe your readiness for change and/or stage of change. How important is implementing your action plan to you? Give reasons for importance. 2. Describe your confidence level and reasons you identified this level. Discuss what you could do to have a higher confidence level. 3. Describe the barriers or challenges you anticipated or actually occurred in meeting your goal. Describe potential solutions to address the barriers. 4. Evaluate your action plan and goal attainment. What worked? What didn’t work? What would you do differently? What do you want to do next or continue to do in relationship to this topic? 5. Briefly identify 1-2 significant learning(s) you had as a result of designing and implementing your personal action plan. (consider motivation, self-management, adherence, self-awareness, use of action plans with clients etc.)
