To learn of some of the practical problems in trying to improve an existing fault-tolerant system, consider the U.S. air traffic control system. Search the Web for information on the current delays, the effects of deregulation, and former President Ronald Reagan’s dismissal of striking air traffic controllers; also see Zuckerman [2000]. A large upgrade to the system failed and incremental upgrades are being planned instead. Search the Web and see [Wald, 1996] for a discussion of why the upgrade failed.
