Allergies NKDA Current Medications
Fentanyl (Duragesic) 75mcg/hr patch Q3days
Sertraline (Zoloft) 200 mg po daily
Lorazepam (Ativan) 0.5 – 2 mg po or IM q4h prn agitation
Risperidone (Risperdal) 1 mg po BID
Phenytoin (Dilantin) 300 mg po QHS
PMH Major depression with psychosis
Hx of breast cancer
Brain metastases

The patient started to hallucinate and became combative with the staff. Her physician ordered 5mg of
haloperidol to be given IM. Later that day the patient came to the nursing station complaining of
