Allergies NKDA Current Medications
Fentanyl (Duragesic) 75mcg/hr patch Q3days
Sertraline (Zoloft) 200 mg po daily
Lorazepam (Ativan) 0.5 – 2 mg po or IM q4h prn agitation
Risperidone (Risperdal) 1 mg po BID
Phenytoin (Dilantin) 300 mg po QHS
PMH Major depression with psychosis
Hx of breast cancer
Brain metastases
1. This patient is complaining that her gums are getting larger and are starting to grow over her teeth.
