associated psychosis. She has a history of breast cancer with inoperable metastases to the brain. NKDA
Allergies NKDA Current Medications
Fentanyl (Duragesic) 75mcg/hr patch Q3days
Sertraline (Zoloft) 200 mg po daily
Lorazepam (Ativan) 0.5 – 2 mg po or IM q4h prn agitation
Risperidone (Risperdal) 1 mg po BID
Phenytoin (Dilantin) 300 mg po QHS
PMH Major depression with psychosis
Hx of breast cancer
Brain metastases
