Part A

1. Select an item  from a freely publically available reusable resource that is also directly relevant to family centred care.

 The  item you choose can take the form of one of the following:

. A genogram .

B-A video clip (up to 5 minutes)
 c. A picture
 d. A photograph of a piece of art
 e. A clip of a piece of music (up to 5 minutes)

F. An audio-recording (up to 5 minutes)


2. Place a copy of the item or embed an electronic link to the item on the first page of your work.  (If embedding a link to your item, ensure you check the link works before you submit your work.)


3. Provide the full reference for the item.

4. Briefly describe the item you have chosen and its relevance to familycentred care. (250 – 300 words)


5. Drawing from a theory relevant to family-centred care, explain ways in which the item can inform care planning as part of a family-centred care approach. Refer to relevant literature throughout your answer.  (450 – 550 words)

 Referring to at least one aspect of significant learning you have gained through studying the family-centred care, how do you plan to improve your practice in future placements? (450 – 550 words) ,use the  article below  in bracket to answer question part B.
