The patient is a 65-year-old
woman who had poliomyelitis at age 3. She sustained complete paralysis of the
right LE and left foot and ankle. During childhood she wore bilateral
knee-ankle-foot (KAFO) orthoses and ambulated with a four-point gait with the aid
of a pair of axillary crutches. When she was 18, she had a left ankle and
subtalar fusion. She was fitted with a right KAFO, which included a stirrup
foundation, posterior ankle stop, drop ring knee lock, knee pad, and
leather-covered calf and thigh bands. For the next 40 years, she wore the same
brace, and had the leather and shoe replaced whenever they became worn. She
used a cane in the left hand when she walked outdoors. She returned to the
rehabilitation department today complaining of pain in the right knee and
fatigue. She also said that her brace tears her stockings at the knee. She is
curious about new orthotic developments.
