The monitoring of the progress of practice is a step ahead of the managed care/quality assurance review process. Single-system designs may have to be clearer in specifying the immediate and intermediate objectives on the path toward some long-term goal. Then, part of the charting and monitoring can be a cooperative process with the reviewer, probably on a random review basis, showing where the client is vis-à-vis a particular objective in a series of objectives toward the goal. With appropriate charting, the practitioner would be ready at any time to convey progress toward objectives, and to record mitigating circumstances if any (as in annotations on the chart). This approach requires that evaluators work with reviewers to reach agreement as to what constitutes the evidence they need.
