Report presentation

The Mental health assessment and intervention report should be well planned.

For this assessment it is important for you to:

  • Outline your introduction clearly and end with a concise conclusion.

  • Make sure you justify your report by referring to what was said during the interview.

  • Write in complete sentences (i.e. do not use dot points).

  • Address the points in Part A as a single section using complete sentences without using sub-headings.

  • Use the points in Part Bas sub-headings to structure Part B of your report.

  • Make sure each sentence only deals with one issue. Keep sentences short, professional and to the point.

  • In Part B the "formulation" section provides a summary of the information contained in the previous sub-sections. New information should not be placed in this section. If it is not in the body of a report it should not be in the formulation section.

  • Does the client need to be referred to a psychiatrist? Why?

  • Pay attention to spelling, grammar and punctuation. It is important to proof read your work before submission.

  • Writing style in a report is more formal than verbal speech. Try not to write as you would speak.

  • Use simple past tense to describe the patient's state at the time of the mental state examination. This emphasizes that the examination findings are specific to the time of conducting the interview.

After you have completed Parts A and B you can start Part C.

Part C requires you to research the mental health literature and recommend three evidence-based interventions to assist the client.

  • Discuss each intervention separately and use a sub-heading so that the reader/ marker know the intervention being discussed.

  • What is the goal for the intervention?

  • What is the clinical issue being addressed by each intervention?

  • Ensure the three interventions you select are consistent with evidence-based treatment guidelines.

  • Justify your decision by referring back to evidence-based literature.

  • Make sure you appropriately reference your work using in-text references to support any statement of fact.

  • Include a reference list on a separate page at the end of the report.

Introduction
In the recent years, mental health has gained increasing importance in the evaluation of the overall well-being of an individual. Governments also have recognised this fact and set aside higher funds to be invested in the study of mental health disorders. The following essay attempts to critically analyse a psychiatric interview conducted on a patient named Andy and find out the probable mental disorder and possible interventions for the same.
History
The video sheds light on the case of a 21-year old boy named Andy. The video pertains to the psychiatric interview for identification of psychosis and related symptoms. Andy appeared to be a confused and perplexed individual (YouTube, 2012). Upon being interviewed, the psychiatrist learned that Andy was in the final year of his degree and that he resided in a shared apartment along with his friends. His main psychological problem revolved around his roommates. Andy reported that his condition was prevailing since the past two months. His mental situation caused him to believe that his friends were contemplating against him.
Andy felt that his friends, who were employees with MI5, were putting things inside his brain. During the investigation process, the psychiatrist observed that Andy appeared very distracted as evidenced by his repeatedly looking at the corners of the room. Upon being asked about the reason behind this, he admitted that he could hear three of his friends talking about him (YouTube, 2012).
He claimed that the voices were very distinct. He believes that his friends are commenting about his actions and planning to throw him out of the apartment. Andy strongly believed that his roommates had installed some chip in his brain to influence his thoughts and track his whereabouts (YouTube, 2012).
 Reason for Referral: Andy’s general physician had conducted the primary examination and referred him to a psychiatrist.
 History of Presenting Problem: According to Andy, he was perfectly fit, with no previous problems.
 Past Psychiatric/Mental Health History: Andy stated during the psychiatric interview that he had no past mental health issues (YouTube, 2012).
 Medical History: According to Andy, he had no health related issues except for the physical pain caused by the installed chip and the associated anxiety.
 Family History: There is no direct mention of Family history in the video. However, the fact that Andy’s mother did not believe him and took him to the doctor may indicate that this mental condition was not known to them.
 Personal History: Andy stated that he had no temperament problems. He would never be involved in any kind of fights with others. It was also clarified that he never faced the police.
 Occupation History: The video does not provide insight into the occupation history of the patient.
 Forensic History: The video does not provide much insight into the Forensic history of the patient except the fact that Andy did not have any criminal records.
 Drug and Alcohol History: During the interview, it came to light that he had no previous history of medicines. He would consume alcohol occasionally. However, he admitted to consuming cannabis and weed (YouTube, 2012).
 Current Treatments: He would have to be given medications for psychosis and related disorders.
 Current Functioning and Supports: The patient would require a three-fold intervention therapy involving psychological counselling, treatments as well as support from friends and family (YouTube, 2012).
 Parental Status and/or Carer Responsibilities: Andy mentioned that although he felt safer at his parent’s place, he did not want to bother them with his disease.
 Details of Dependents: As...
