Task Rationale: Community nursing requires nurses to acquire the skills needed to develop and implement health promotion activities for priority areas and priority groups. This requires skills in understanding the health needs of a group, considering how to reach the target audience and propose appropriate intervention that is relevant, accessible and cost-effective. This written assignment will address course learning outcomes 1, 2 4 and 5. Task Description: 1. You are a community nurse working in a community health centre. Choose ONE of the following options which will provide the context for you to describe a problem / unmet community need:   Refugee health for refugees from Afghanistan  Cardiac rehabilitation, including lifestyle management, for men over 50 years following a myocardial infarct Rural or remote defence-force community young mothers If you would like to choose your own topic, you must consult with your campus course convenor. Please come prepared to outline the following:  The unmet community health need and the type of community with this need. 2. Write a 2000 word project proposal. In your proposal you must:   Describe the aims of your project (approx 100 words)  Describe the problem or unmet community health need that your project proposal will Create a meaningful title for your project proposal (less than 30 words) address and clearly contextualise the problem / unmet community health need for the identified group (approx 750 words)  Describe the proposed health promotion program / activity (approx 150 words)  Describe the resources you will need to implement the health promotion program / activity (approx 100 words)  Justify the benefits and relevance of your proposed health promotion program / activity, including how it will address the problem / unmet community health need. Your justification must discuss relevant community nursing concepts for example advocacy, equity, self- management, health literacy, and engagement. Use literature to support your rationale, this can include current policies, frameworks, and relevant legislation. (approx 750 words)  Describe how the aims of the project will be evaluated (how you will assess whether you achieved the desired project outcomes) ( approx 150 words) OTHER ELEMENTS: o Always refer to the Griffith Health Writing and Referencing Guide o Ensure that you use scholarly literature (digitised readings, research articles, relevant Government reports and text books) that has been published from 2008 onwards. o Word count suggestion- see marking criteria for suggested word count. You do not need to provide a separate introduction or conclusion to your proposal. o Unless otherwise instructed, write in the third person. o You may use headings utilised in the marking criteria, to organise your work, if you wish o Do not exceed 2,000 words. Words over 2,000 will not be marked. o Use academic language throughout. o Refer to the marking criteria when writing your assignment.
