Scenario 4: Burril is a 72 year old Aboriginal Australian living in Wamba Wamba Nation near the Murray River some 65 kilometers from Swan Hill. Burril has an existing diagnosis of Rheumatoid Arthritis and was recently diagnosed with advanced Alzheimer’s Disease following a hospitalisation for severe pneumonia. Conversations with his family suggest that Burril had been showing signs of his dementia for close to a decade. Burril is quite frail and dependent on the care of his family. Burril exhibits loss of memory and inappropriate behaviours. An indigenous health worker suggested that the family had not sought any medical help for Burril because they did not want Burril to be taken from his country and had considered the symptoms a sign that Burril had a sick spirit. Burril is an elder in his community and as such is an important story teller. The loss of Burril’s memory is felt by the broader community. Burril is now con sidered to be entering a palliative stage of care. Burril’s family remain adamant that Burril would not want to leave his country although conversations with Burril himself have been limited. Burril’s community, however, is remote and lacks local healthcare resources. Since their discussions with the indigenous health worker when Burril was hospitalised, the family are open to exploring all avenues of medical help to optimise Burril’s comfort and quality of life and so the community can keep their elder for as long as possible. The family, however, feels strongly about Burril remaining at home. Topic Your group will present considerations for a collaborative discussion. This can be a Power Point presentation. Length 12 minutes (this includes 1- 2 minutes question time) Presentations must be more than a minimum of 10 minutes and no more than 15 minutes as a maximum. Value 2 0% of the total marks for HLT1IPP Due date D uring your Week 12 workshop . The aim of this assessment is to introduce you to collaborative practice as it relates to the provision of services to consumers. This assessment will be graded in your workshop in week 12 by your facilitator. The purpose of the oral team presentation is to present considerations for a collaborative discussion regarding a case scenario. Each team will have a different scenario that will be assigned by your workshop facilitator. Your presentation should: 1. Identify who the primary client/ patient is 2. Describe the main issues or concerns for the client/patient in the scenario 3. Identify the types of health and human service professionals that would be involved in the interdisciplinary team/ case conference for this scenario 4. Identify the expertise each member of the interdisciplinary team brings to the interdisciplinary team / case conference 5. Identify the questions, considerations or concerns each member of the interdisciplinary team would have in relation to this scenario 6. Identify a number of available care options that are appropriate for this scenario 7. Nominate, with justification, a preferred care option for the client / patient 8. Comment as to how effective collaborative practice would be achieved in this scenario Length: 12 minute s (this includes 1 - 2 minutes question time). Marks are awarded in the assessment rubric for presentation duration . Please note that your presentation is more than 10 minutes as a minimum and no more than 15 minutes as a maximum. All students are expected t o be in attendance for their presentation and that of their fellow students. This group presentation should be approached collaboratively and prepared based on the contributions of all members of the team. It is unlikely that a good result will be produc ed if sections are divided up among team members and assembled without collaboration. You will be given time during workshops to work on the presentation, but you are also expected to work together on the presentation in your own time to ensure the project is fully completed before presentations are held in the Week 12 workshop. Marking: This is a team assessment. There is no provision for an individual student to receive a mark higher than that of their team’s grade. All team members who have contributed will receive the team score. Students who do not contribute to the planning, development or delivery of the presentation will not receive the team mark; a score of '0' will be entered into gradebook.
