Scenario 2 Mr Benjamin Heggarty (D.O.B - 11/08/1942) attended your clinic today and he is a regular INR client on Warfarin Therapy for Atrial Fibrillation (AF). His current dose of Coumadin is 5mg Monday, Wednesday, Friday and 4.5mg the rest of the week. Mr Heggarty states that he missed his dose by mistake 2 nights ago and that he has started taking Panadeine Forte (2 tabs morning & night) 4 days ago for back pain. Mr Heggarty also reports that he had a nose bleed last night that lasted for 5 minutes
