Introduction
Primary Health care (PHC) also known as essential healthcare is an important concept with respect to the access of healthcare facilities for individuals in the society. Principles of PHC are required to be based upon scientific grounds along with socially acceptable technology or methodology. The concept of PHC is to make universal healthcare accessible to every individual in a community (Hum et al., 2017). This essay will focus upon the PHC promotion strategies for the diabetes among old age individuals. There have been no specific health promotion strategies to emphasize on diabetes mellitus.
Therefore, Australian National Diabetes Strategy 2016-2020 is one of the recent and most effective health promotion plan implied. This assignment will focus upon outlining major goals and principles of this strategy along with the community involvement. Previous assignment discussed the issue with demo-geographics; therefore, this assignment will be based upon PHC promotion strategies for prevention and control of diabetes mellitus among old age individuals in Australia. The target groups for this public health promotion would be group 2, which would be old age individuals above 65 years old living in remote areas of Kimberley, Australia.

As discussed in the previous assignment also, old age individuals form up a major proportion of Australian community. According to the data presented by census, 2017 one in every 7 individuals were aged 65 and above. Therefore, this constitutes to a population of 1,188,989 individuals aging above 60 in Australia and this number is expected to increase with the growing population (Australian Institute of Health and Welfare, 2018).
It would be essential to consider Kimberley lies in the north most region of Western Australia and is also considered as the most earliest part of Australia. According to the data provided by 2016 census, 41.6% population of Kimberley comprises of Aboriginals and Torres Strait Islanders communities. People aged 65 years and above makes up 6.3% of Kimberley’s population. Literacy rate as well as modern health facilities in the remote areas are very less; therefore, majority of the target group 2 population does not possess health literacy. As a result, health promotion plan becomes a necessity for the indigenous old age individuals as well as their family members.
Harding, Shaw, Peeters, Guiver, Davidson and Magliano (2014) mentioned and emphasized upon the fact that diabetes is a chronic issue and is characterized by the presence of high glucose levels in blood. There could be several causes for diabetes and these include genetic as well as acquired factors. Body’s inability to synthesize appropriate levels of insulin is considered as one of the major issues behind the existence of diabetes mellitus. According to the data presented by Australian Bureau of Statistics (ABS) 2014–15 through the National Health Survey conducted, 1 in every 6 individuals over 65 years of age were reported to have diabetes and this contributes to an overall population of 574,000 older people with diabetes in Australia (Trump & Mendenhall, 2017).
Huo et al. (2018) mentioned and argued upon the fact that diabetes mellitus has been characterized as type 1 and type 2. Treatments such as, insulin therapy are available for type 1 diabetes while there are no significant or effective treatments available for type 2 diabetes. The Department of Health (2011) presented that the prevalence of type 2 diabetes among 65-70 year old individuals, irrespective of their gender have been increasing. Out of all the self-reported cases, it would be essential to consider that 83% cases were reported to be of type 2 diabetes mellitus while only 13% were from type 1 diabetes while 4% of individuals were not even aware of the type of diabetes they had.
Australian Institute of Health and Welfare (2018) presented that among indigenous population prevalence and existence of type 2 diabetes has been reported to increase with an alarming rate. Major causes of rising indigenous population with type 2 population have been reported because of the lack of physical activity and increased rates of obesity. Type 2 diabetes is not a life threatening disease as it accounts for only 10.4% deaths annually. Death due to type 2 diabetes is not due to direct causes rather, indirect effects on other body organs such as kidneys and heart. Therefore, promotion of causes as well as prevention strategies for type 2 diabetes would be an essential task under this project (Carrasquillo et al., 2017).
Project Description
Prevalence of diabetes among native population has been reported to be almost double than the non-indigenous population. Two in every five indigenous individuals above 55-60 years of age have been reported to suffer from type 2 diabetes (Lee, Ng & Chin, 2017). For type 1 diabetes, total annual cost in Australia has been reported to be around $570 million while the average annual cost per person has been reported to be $4,669. Therefore, this data clearly reflects the need for PHC promotion and self-care promotion strategies for controlling diabetes mellitus in older Australian population (Australian Department of Health, 2020). There have been numerous programs launched by the authorities in Western Australia and they provide health promotion against diabetes to the families or individuals diagnosed with diabetes. However, it would be essential to consider that a very few number of individuals visit the diabetes clinics to be tested. Mostly native communities do not prefer to visit or receive medical care; therefore, the health promotion programme is required to target individuals that might not even be aware of being tested for diabetes.
