A 31-year-old woman is admitted to a hospital ward with severe acute pancreatitis and develops a splenic infarct caused by a portal vein thrombus in her liver. She has a history of repeated presentations of acute on chronic pancreatitis caused by familial hypertriglyceridemia. She also has type 2 diabetes treated with insulin. During her admission, she develops a gastric outlet obstruction, multiple liver abscesses, ongoing sepsis, chylous ascites and diarrhoea. She has an extended admission with gradual decline in her body weight going from 88 kg to 45 kg in 6 months and is now considered malnourished.
