Purpose The purpose of this assignment is to enable the student to draw together and demonstrate their understanding of key aspects of nursing and nursing care for clients in an acute/complex setting with a particular focus on the needs of Māori. Ethical Consideration Consideration of the impact your inquiry may have on your chosen client is very important. Speak with your preceptor first. Requesting permission to access the client’s file to assist with your assignment, is an ethical issue. Gain informed consent. Use pseudonyms to protect the identity of the client, the clinical setting, and other people involved in this assignment or any other identifiable information. The signed consent copy will be placed on the client’s clinical file. You must attach a blank copy of this consent form as an appendix in your assignment. Your clinical lecturer will need to sight the signed consent. Assessment Task 1. Choose a client you are working with who is experiencing an acute/complex health condition who you can complete a case review for. Wherever possible choose a Māori client. 2. Develop a consent form. Attach a blank copy as an appendix. 3. Describe the aetiology (contributing cause/s) of your client’s presenting acute/complex health condition. 4. Use the Te Whare Tapu Whā health tool to complete a comprehensive assessment of your client. Attach the completed Te Whare Tapu Whā tool as an appendix. 5. Describe the impact this health condition has had on your client from your client’s perspective. 6. Evaluate your clients knowledge in relation to Whānau Ora and how they feel Whānau Ora can support them 7. From your nursing perspective, discuss with your client how Whānau Ora can inform their health outcomes. 8. Concisely, describe the Pathophysiology relating to your client’s presenting acute/complex condition. 9. Choose one nursing assessment, which you have performed on your client (e.g. MSE, pain assessment, respiratory assessment) relevant to your client’s presenting acute/chronic health condition. 10. Describe in depth the nursing assessment you performed on your client, include your rationale for each step of the assessment. 11. Complete a literature review on the selected nursing assessment. Compare and contrast the literature findings. 12. Reflect on whether the nursing assessment you performed on your client is evidence based. Identify any areas your nursing practice can be enhanced as a result of the literature review.
