Abby got fired from her job last month, which was her only means of income to support herself. Since then she has remained in her bedroom and cries on more days than not. Her appetite has decreased substantially, resulting in significant weight loss. She has considered buying a gun, but cannot afford it and does not have the energy to leave her apartment. 2. Sarah arrived at her therapy session in tears, saying that she awoke this morning in bed with a strange man and had no memory of the previous night. As she described what she could remember from earlier in the day, Sarah’s demeanor suddenly became more flirtatious and seductive. Previously Sarah, now introduced herself as Amy and explained that she met this man at a bar last night and went home with him. 3. Kevin has always known he was different from other people. He does not feel emotions that others seemed to feel (especially about people) and so he looks to others for how he should behave in situations to fit in and go unnoticed. He does not want to attract any undesirable attention to himself. His role in the sexual slavery business is very lucrative and he does not want to jeopardize that. Kids and adults alike, he does not care so long as he gets paid. 4. James lives on the 7th story of his apartment building. He always climbs the stairs and counts each step as he goes along. When he arrives at his apartment door, he rotates his key in the lock seven times before opening the door. Once inside, he rotates the deadlock seven times and touches the doorknob seven times. 5. Mat is frequently in trouble at school. He is constantly getting into fights with other kids at school. He does not listen to his teachers and has trouble staying focused the lessons each day. He seems to be a smart kid, capable of the work, but his energy is directed wherever he looks at that moment. 6. Abby’s boyfriend of 3 years broke up with her just after their college graduation. She has tried to understand why, and believes that her weight gain was one of the many reasons. Despite his now being hundreds of miles away in a different state, Abby recently began believing that he is randomly flying back to spy on her and poison her food. 7. Josh becomes very nervous whenever he is around people. He is always worried that he will say or do the wrong thing and those around him will ridicule him. He does everything he can to avoid people or endures it with significant distress. 8. Kelly will not leave her house when it is dark outside. She is always on alert for danger when she is in public and often overreacts to people being close to her. Last year, she was volunteering in Panama City when she was kidnapped. She spent three days with her captors enduring abuse before they dropped her off on the side of the road. She feels grateful to be alive but still blames herself for not being more careful. 9. Jamal thinks very highly of himself. He unapologetically tells his therapist that he surrounds himself with only the most intelligent and prestigious individuals because only those people can understand and appreciate him. However, he talks about his own accomplishments and admirable qualities so frequently that a lot of people find him difficult to be around and he is quite lonely. 10. Carol has lost three jobs recently and is jeopardy of being homeless. About six months ago, she felt like she had enough energy to stay away for days and solve the world’s problems. She regularly tried to contact the Pentagon to share her theories on nuclear development and spent a lot of money to travel there for several days in the middle of the week. It didn’t take long for her to realize that she spent all of her money on this useless trip. She realized she is not smart enough to solve any world problems – she cannot solve any of her own problems. She has trouble getting out of bed most days, eats a significant amount of unhealthy food, and believes life is hopeless. 11. Katie was in the middle of the grocery store today when she began sweating. Her heart was pounding, she was short of breath, her thoughts were racing, and it felt like she was going to die. She immediately left and has significantly limited her time outside the house since then. This was the third unexpected event like this and she doesn’t want it to happen again. 12. Jackie has had a long history of chronic uncertainty about what she wanted to do in life and with whom she wanted to be friends. She had many brief, intense relationships with both men and women, but her quick temper led to frequent arguments and even physical fights. 13. Calvin loves trains. He wants to play with trains and talk about trains and dress like a train conductor. He does not make eye contact with others and his speech is limited. He screams and yells when someone tries to take away his trains. He needs assistance to complete hygiene and other daily life tasks. 14. Gale has not invited anyone over to her house in years. She is embarrassed by the amount of stuff that she has accumulated. It is not necessarily worth anything, she knows this, but she cannot get rid of any of it: it might still be useful. And that is what she thinks every time she passes something on the side of the road. She feels compelled to take it home.
