U.S. Department of Health and Human Services Initiative to Eliminate Racial and Ethnic Disparities in Health (1998) • To reduce disparities in six key areas: infant mortality, cancer screening and management, cardiovascular disease, diabetes, HIV/AIDS, immunizations 1. The name of the program and the source of your material---the website address, name and date of newspaper article, etc. 2. One fact about the program. You may choose to report one of the following facts about the program: --Past accomplishments (for example, the number of families served or clinics opened in a year), --Progress reviews for an ongoing program, (for example, 10% increase in seat belt usage, from 60 to 70%, over the first five years of a 10-year program) or --Future projects (for example, grant applications are being taken from rural hospitals who will participate in a study about migrant worker health issues) 3. Your opinion as to whether you think the program you chose would help underserved populations in your community or at your place of employment. You may have chosen to write about a program that you have seen in place at your worksite. If so, summarize the results of the program that you have actually observed on the job. 4. The reasons for your opinion as to whether the program would help or has helped the underserved in your community or not.
