Prepare a case study for 200 words. This should be a case discussion of a person or family member receiving health or social care and known to the student- *such as -someone who has been diagnosed of mild learning and Schizophrenia. Using relevant theory the student should critically discuss the relationship between themselves as practitioners and the service user. Discussion may also include other relevant social care practitioners such as Community Practice Nurse, Learning Disability Psychiatrist, Doctor etc. ( Here discuss care given). The case study should have a gap in service provision among the professionals such as inequality, discrimination, Not giving holistic care to the service user. It could be in hospital, care homes supported housing accommodation.

Case Study: Working with someone with learning disability and schizophrenia.
Mr X a 49years old, who is well known to me, had been living in Supported Housing Accommodation in the community for many years. He has a diagnosis of mild learning disability (LD) and schizophrenia with irregular input from learning disabilities team. He was reviewed quarterly by her Social Worker and yearly by her Consultant Psychiatrist.
Recently, there are concerns from his support worker where he lives around self neglect, poor sleep, weight lost and behaviour. Mr X was found lying in his own faeces within his flat. The support worker informed his social worker for him to be assessed and with review undertaken by his Community Practice Nurse (CPN) and LD Psychiatrist.
The support worker noticed there was  slow support coming from the professionals because of Mr X history of rape and violent towards women. Few days later, Mr X was found collapsed in his flat and rushed to hospital. His admission was uneventful and remained stable but a new placement was required due to his evident deterioration in his self help skills.
Eventually,  Mr X been discharged and wanted to sue the professionals for  failing in their responsibilities. Prior to this, his multidisciplinary teams felt that he required full assessment and a possible new care package to meet increased needs.


Assessment TASK B
The case study will then be used to produce an essay for 2000 words which should demonstrate an understanding of how to work in partnership with service users to provide holistic care. Discussion should cover issues arising in the case study and such issues of discrimination, inequality in society and user involvement.


The essay should specially cover:
a. An understanding of the social and psychological factors that are relevant to consider in their chosen case.
b. A discussion of how they worked in partnership with the person /people in their case example
