NURSING CARE AND PATIENT SAFETY 1
Assignment
Part A:
The role of a nurse is significant when taking a decision of providing medicines to the patients with the maintenance of the healthcare records of the patients. The aim of handling medication s
afety for the patients is to ensure that there is proper coordination, communication, and interaction between the nurse and the patient to ensure a speedy recovery and reconciliation by the patient undergoing treatment. Further, another aim for the nurse is to form a link between a healthcare provider and a physician so that the right medicine can be provided to a patient undergoing treatment on time (Mardani et al., 2020).
Keywords: efficacy, medicinal safety, quantitative, exclusion criteria, inclusion criteria, reconciliation terms, and database.

Part B:
1. Citation:
1. Mardani, A., Griffiths, P., & Vaismoradi, M. (2020). The Role of the Nurse in the Management of Medicines during Transitional Care: A Systematic Review. Journal of Multidisciplinary Healthcare, Volume 13, 1347–1361. https://doi.org/10.2147/jmdh.s276061.
Main point:
The PRISMA analysis was performed in this nursing care intervention with the identification of the exclusion and inclusion criteria. The inclusion criteria included the type of care provided by a nurse to a patient in the hospital and after discharge at home (Mardani et al., 2020).
Evaluation:
The study included the option of undergoing treatment by the patient for a short-term period and for a longer time period. The exclusion criteria included those articles in the database in which the plan of treatment was not specific and if the same data was repeated several times with redundancy (Mardani et al., 2020).
Support:
The quantitative approach includes providing the right care at the right movement and thus reducing the readmission and the revisit chances of the patient to a hospital setting a number of times.(Mardani et al., 2020).
Reflection:
Older adults in society suffering from mental abnormalities like dementia need special care, pamper, support, coordination, and communication to overcome their problems in life (Mardani et al., 2020). Nurses need to follow the nursing ethics and regulations.
2. Citation:
2. Prusaczyk, B., Fabbre, V., Morrow-Howell, N., & Proctor, E. (2020). Understanding transitional care provided to older adults with and without dementia: A mixed methods study. International Journal of Care Coordination, 23(1), 14–23. https://doi.org/10.1177/2053434520908122
 Main point:
The main point of this article is that a survey was organized in which different older patients suffering from the disease dementia were monitored by analysing their medical records to check whether all the data is present in the files or not and whether patients are provided the right medication under proper supervision in the hospital or not? (Prusaczyk et al., 2020).
Evaluation:
The emphasis is on the main point that there are different roles of the healthcare managers, nurses, physicians, registered nurses and advanced practicing nurses while treating, taking care, and while discharging the patients in a hospital...
