O/A - Heart rate at 110, Blood Pressure of 165/85, Respiratory rate at 21, Oxygen Saturation of 97% on Room Air, Temperature of 37.2 celsius, BGL 10.8mmol/L.




Demographics


John Smith is a 66 year old male, presenting with Tachycardia, Chest Pain and Shortness of Breath (SOB).




Past Medical History


John was diagnosed with Coronary Artery Disease and Stable Angina in 2008, currently managed with Atorvastatin BD and Glyceryl trinitrate PRN. History of type 2 diabetes, managed through diet. Further medical history of Hypertension, Hypercholesterolemia and Obesity.




Social History


John Lives with his Son James, his daughter in-law Jane and his 3 grandchildren. He is ambulatory with the assistance of a 4 wheeled walker however has a sedentary lifestyle spending most of his day at home, rarely ambulating outside to smoke with Nil physical exercise. John is an ex-smoker having quit 20 years ago, when he did smoke he smoked 10 Cigarettes a day. John only drinks alcohol socially once a week. John is currently independent with his care, with his son buying and making his food for him when he can, however Johns diet mostly consists of high fat fast food.
