1.    Read the needs assessment and answer the following questions cutting and pasting relevant statements and/or using dot points where appropriate.

a.    How is the community in the needs assessment formally defined? If a standardised definition is not used, propose one that you will use to source additional data for this community. (1.5 points)
b.    What type of community is it – geographic area, social place, community defined by demographic characteristics, or demographic-ethic community? (0.5 points)
c.    What was the purpose of the needs assessment? Is it an example of a good purpose statement? If not, why not? (1.5 points)


PART 2

2.    Complete the table below using a health determinants model for your selected needs assessment.
a.    Using a determinants model, complete the “Level of determinant” and “Health determinant” columns with relevant information.
b.    List all the health and wellbeing measures/indicators reported in your needs assessment in the appropriate column along with the corresponding reference/s.
c.    Identify and indicate with an * determinants not reported in your needs assessment.
d.    Identify one (1) relevant additional measure/indicator  for each of the following determinants, indicating them with an *:

i.    Social health status or social behaviour
ii.    Policy environment
e.    Source data for the indicators/measures identified in 2d and record the source of the data in the "Data source" column, stating the source agency, title of data, date of data collection and website link.
