Aim of assessment


The purpose for this case study essay is to enable students to produce a nurse-led case discussion of an older adult, using evidence-based information relevant to nurses and up-to-date Australian statistics, for improving the health and quality of life for elderly persons who have multiple comorbidities and care needs. Content should include discussion on the questions provided in the case study using a holistic approach.








Case study


Maria Russo isan83-year-oldlady with a rich Italian background. She migrated to Australia when she was 51 years and ever since has lived and worked in Australia, she is a widow who lives alone in her unit and has been independent with her activities of daily living. She has two daughters Gianna and Bianca who live nearby and are fully involved with the care of Maria. Mrs Russo presented to emergency department (ED) via ambulance following a fall in her garden. She had fractured her left NOF and has been to operation theatre (OT) for total hip replacement (THR). Mrs Russo is now day 7 post op.


Mrs Russo has a medical history of osteoarthritis and had a major stroke5 years ago but managed to recover well after months of rehabilitation. She has moderate congestive cardiac failure, a mild cognitive impairment, NIDDM for ~ 20 yrs, with pain associated to neuropathy. She has had two recent transient Ischaemic attacks (TIAs)at home and managed to recover well after few days of staying in bed. Since she lost her husband, Mr Russo 7 years ago, she cycles through depression if she forgets to take her antidepressants.She wears glasses for her vision impairment related to glaucoma, cataracts, and diabetic retinopathies. She also wears bilateral hearing aids.


During her stay in acute careward, the nursesdocumented in her progress notes that Mrs Russo is:


– Underweight with a BMI of 14


– On admission looks quite dishevelled and dehydratedindicating possible neglect and poor care


– Very frail with multiple skin tears on her lower limbs and stage 1 pressure injury to her sacrum


– Old scars to lower legs with possible history of healedvenous/arterial ulcers


– Might have been on the ground for over 8 hours before a neighbour found her


– Denies pain, but noticedto be grimacing and wincing during transfers and repositioning


– No recollection of memory of her having used her bowels 4 days before she presented in ED


– Developed delirium post-surgery that resolved after treatment and hydration



After nine days in acute care ward and three weeks in rehabilitation ward, Mrs Russo was not able to return home and care for herself. After a long discussion with multi-disciplinary team (MDT), her daughters Bianca and Giana reluctantly agreed to transfer Mrs Russo to an aged care facility where she will receive 24hr care. Health education during discharged included: medication compliance, hydration and nutrition, bowel management, skin care, prevention of skin tears as well as future advise on palliative care and endof life care.

This fact has an enormous impact on the country’s health care provision system because of the increased number of people with various geriatric ailments and morbidities. The increased number of aged populations implies the need for multiplication of the medical and paramedical staff and the installment of various advanced medical and laboratory equipment. 
THE STATISTICS OF CONTEMPORARY AGEING IN AUSTRALIA AND THE IMPACT OF THE AGEING POPULATION TO THE HEALTH CARE SYSTEM
Old aged people have many age-related diseases, and conditions specific to their population, such as hypertension, ischemic heart disease, stroke, non-insulin-dependent diabetes mellitus, osteoarthritis, and osteoporosis, cataracts, hearing loss, dementia, anxiety, depression, and the list goes on. Most of these ailments result in physical or mental disability and require prolonged admission in the health care providing institutions. These patients need specialized supporting care and rehabilitation in addition to the prescribed medication and, hence comes the role of nurses and paramedical staff. Nurses play a crucial part in implementing the management plan advised by the specialists; they might be the backbone of a particular health care system. As per the devised guidelines for nursing, geriatric care by The Australian College of Nursing, the nurses should have ample knowledge and understanding of the physiological aging process and the challenges it presents, so that they can provide essential medical care along with psychological support to the old aged people and their families ("The role of nurses in promoting healthy ageing", 2019).
In light of the increasing senior population in Australia, Geriatric nursing care also needs to stay up to date and capable enough to nullify these percentages’ negative impact on the country’s economy. The mental and physical handicap of aged people can put an enormous burden on society as a whole and the individual level. Hence, Nurses have to play a significant role in promoting their general wellbeing and ensuring healthy aging by making them able to meet their basic needs. The competent nurses in Australia should recognize the acute and chronic co-morbidities of the older adults and the delivery of associated treatment hence maximizing the health outcomes.
IMPORTANCE OF HEALTH PROMOTION IN THE CARE OF OLDER ADULTS
Health promotion is an integral part of a successful health care system, and it is particularly important in establishing geriatric home-based care. The general health and wellbeing of older adults depend on multiple factors and should be addressed using a multidisciplinary approach. For instance, the old lady mentioned in the case study had various health issues, including physical and mental aspects, and thus, her family required detailed counseling regarding her care at home. Older adults are physically weak due to physiological wear and tear of the body that affects their mobility, putting a hurdle in carrying out daily life activities. They might have specific visual and hearing problems, making it difficult for them to lead an everyday life without external help. Additionally, the prevalence of dementia and memory loss increases during the later stages of life and contribute to a further decline in older adults’ quality of life ("Dementia", 2019). Hence, the caregivers and family members should properly be educated and counseled about the risk factors of common geriatric illnesses and the treatment and rehabilitative modalities. Only through proper education of family members and training of the caregivers’ staff, it is possible to improve older adults’ quality of life and thus lighten the enormous burden they put on a state’s health care system.
