Nora is a 14-year-old female referred to you for medical nutrition therapy by her primary care provider (PCP). Nora has gained over 20 pounds this past year. Her PCP is concerned that with continuous weight gain and high low-density lipoprotein (LDL)/triglyceride levels, Nora will develop type 2 diabetes and chronic cardiac disease. Nora came to her initial appointment with her mother, who is morbidly obese. At the initial meeting, Nora explained that her favorite foods include fast-food options and processed desserts such as Yodels and cinnamon buns. Nora does very little physical activity during the week and claims that she does not find any enjoyment from exercise.

Adiposity consistent with android body type. Oral exam, hair, and nails appear normal. Skin is warm and intact. No evidence of acanthosis nigricans. Abdominal exam unremarkable. No evidence of lower or upper extremity edema.
