He presents following two days of right upper quadrant (RUQ) abdominal pain, fever, and vomiting.The previous nurse hands over to you: HR 126, BP 100/45, Temperature 38.8, severe RUQ pain with the last episode of vomiting 2 hours ago. The nurse also states that he is pale with dry mucous membranes, and that he is requesting a drink of water. He reports shoulder tip pain scored at 7/10.
