Mr. Kasim Al-Mutar is a 49 year old man who presents to the emergency department with cholecystitis. He presents following two days of right upper quadrant (RUQ) abdominal pain, fever, and vomiting.

The previous nurse hands over to you: HR 126, BP 100/45, Temperature 38.8, severe RUQ pain with the last episode of vomiting 2 hours ago. The nurse also states that he is pale with dry mucous membranes, and that he is requesting a drink of water. He reports shoulder tip pain scored at 7/10. Kasim’s 12 year daughter is with him.
