Monitor and Evaluate
1. Uncontrolled painful diabetic neuropathy
■ Improved symptoms based on pain levels, improved function and quality of life, improved sleep quality.
■ Signs and symptoms of side effects upon initiation and slow titration of gabapentin: daytime sedation, confusion.
■ Improved haemoglobin A1C every 3 months following adjustments after follow-up with PCP for diabetes mellitus (DM) management, attendance of diabetes self-management skills class.
2. Constipation
■ Bristol stool chart and signs and symptoms of constipation (signs of straining, incomplete evacuation).
■ Frequency of use of polyethylene glycol 3350 to determine whether this should be scheduled.
3. Hypertension/dyslipidaemia
■ Continued control with current regimen, no reports of adverse drug reaction (ADRs).
