Introduction
The given scenario explains the physical and mental state of the patient. Shanae is a 22-year-old woman who has been admitted to the hospital with multiple physical and mental issues and assessed by the acute care team nurses. The appearance and behavior and overall history of personal as well as professional life of Shanae
have been judged carefully. The self-harm and suicidal attempts of Shanae tell about the dysregulation of emotion and mental instability. Disturbance in the personal, as well as social life has also spread a negative impact and led Shanae into a long term depressive phase. The social and mental background of the patient tells about various distressing and traumatic events that can be considered as a major agitator for this symptomatic behavior of the patient. Abnormal experiences and uncertainty in professional life have influenced Shanae to incorporate various abnormal behaviors in her personality along with a huge reduction of self-regulation skills. After a multi-dimensional judgment of physical and mental health from every possible way, Shanae has been diagnosed with a border personality disorder. In this particular assessment, a treatment plan for the care of Shanae has been discussed in a detailed point of view.
Trauma informed care
Generally, trauma-informed care has been involved with the entire health care system (Chaudhri, Zweig, Hebbar, Angell, & Vasan, 2019). The health care professionals follow the background of the disease along with the history of the professional and personal life for a better understanding of the situation.
Understanding the nature of trauma, the promotion of the environment for the individual patient to improve the healing or recovery rate is the main part of the trauma-informed care. Abusive and adverse environmental impact along with the adverse childhood experiences can have a major impact on the physical as well as emotional health of an individual and that increases the dysfunction and inflammation (Gill, et al., 2020).

The history of intentional self-injury and several overdoses of the patient has been recorded and the cause of long-term depression and as well as the situation of the daily struggle of Shanae has been revised. Acknowledgment of the traumatic experience of the patient is needed to be justified properly as an initial step of trauma-informed care (Strait, & Meagher, 2019). The experience of the patient has to be listed in detail and proper supportive behavior is to be shown to them to make them comfortable. In the case of, she has been brought to the emergency department with a situation of an intentional overdose of paracetamol along with self-inflicted carts to her both wrists. Her former boyfriend has stated about the situation and described her text messages and confession. All the good and bad situations and stories of childhood are to listen to initiate a better quality of treatment.

The self-destructive behavior of Shanae and explosive anger make her upset and depressed. Feelings of emptiness and anxiety reduce the tolerating power and make her incapable to manage her emotions (Buck, Vertinski, & Kukla, 2018). The misuse of alcohol and overdose of other drugs has created adverse effects on the physical and mental health of Shanae.
On the discussion of the current scenario, it can also be observed that initially Shanae was cooperative with the police but she seemed less cooperative with the emergency examination authority. As a part of the mental state assessment process, the appearance and behavior of Shanae have been recorded. It has been recorded that Shanae was a softly spoken woman and her mood was depressed. In the case of auditing her thoughts and cognition Shanae revealed her fascination toward children and animals. Shanae has a perception of hearing the voice of an unknown man when it is dark. Overviewing all the impressions of the patients it can be predicted that he is suffering from a border personality disorder.

To maintain mental health it is also needed to maintain a personal and professional life as well. This given assessment is generally focusing on the recovery planning of acute mental health disorder. From the overall description of the concerned patient, the special mental disorder has been identified based on the sign and symptoms the diagnosis plan has also been developed. The therapy of this specific mental health disorder generally aims for the emotional recovery of the patient (Derks, Klaassen, Westerhof, Bohlmeijer, & Noordzij, 2019).
Intervention
A borderline personality disorder is one of the most common mental illnesses in the psychological fields. The exact cure process of this disease is still unknown but various effective paths are being followed by the professionals to reduce the signs and symptoms of the disease (Grenyer, Lewis, Fanaian, & Kotze, 2018). It is also being seen that through the continual process of medication and psychological treatment effective improvement of the patient's physical and mental health is showing a new path of the treatment program (Rao, Heidari, & Broadbear, 2020). The vulnerability of the disease can be seen in early adulthood and it remains present in a variety of contexts. Here in this particular scenario, the personal and professional, and social background of patient Shanae has been observed. Overviewing the overall physical and mental situation of the patient a suitable well-being assessment and intervention plan has been developed to promote the care process of getting a fruitful improvement result of the patient's health.
