Mr. dela Cruz is a 67 year old male who has been admitted to the Medical ICU with a diagnosis of congestive heart failure (CHF). He is intubated and attached to a ventilator, with nasogastric tube attached at the L nostril, intravenous fluid (IVF) of D5 LR 1L x 12 hours, and foley catheter draining to a pale straw urine. He is monitored hourly for his vital signs as well as intake & output. Nurse Tina was assigned to take care of Mr. Dela Cruz and the following tasks transpired during her morning shift:
After endorsement, Nurse Tina made her rounds to check on Mr. Dela Cruz. She took his vital signs, level of consciousness. His present IVF has a remaining amount of 600 cc & regulated the desired rate. She prepared the medications and osteorized tube feeding (OTF) all due at 8 am. She fed Mr. Dela Cruz with 200 ml of OTF and instilled 50 ml of water afterwards. She administered 2 cc of Furosemide 40 mg IV. Dr. Cruz made his rounds and made his new orders for Mr. Dela Cruz. At 10 AM Nurse Tina emptied Mr. Dela Cruz urinary bag and obtained 350 ml of urine. At 12 noon, another 200 ml OTF was given to Mr. Dela Cruz with 50 ml of water after feeding. Nurse Tina administered 5 cc of his due antibiotic and another 250 ml of urine was drained.
At 2 PM, Nurse Tina is preparing for her endorsement for the incoming afternoon shift. She took Mr. Dela Cruz latest vital signs, level of consciousness and checked for the remaining IVF which is 200 ml and emptied another 200 ml of urine from the urinary bag.
