This information which is in the Student Assessment pack is to be handed to each student to outline the assessment requirements
Student Instructions
This assessment is to be undertaken in a fully equipped a fully equipped disability support care facility.
For this assessment, you are to be observed on 3 separate occasions (2 observations by your workplace supervisor and 1 observation by your assessor).
Part A
For this task you are required to demonstrate your ability to provide positive support in response to at least 3 different situations of behaviours of concern. You are to perform the following tasks:
a. Identify, recognise, manage challenging behaviours or behaviour of concerns in the context of your job role.
b. Identify the causes or triggering factors of the challenging behaviours or behaviour of concerns.
c. Identify the support needs of the individual.
d. Support the client with challenging behaviours or behaviour of concerns professionally according to the policies and procedures of the organisation
e. Complete the reporting requirements according to the policies and procedures of the organisation
Part B
For this task you must demonstrate your ability to:
a. Support the person to maintain their activities of daily living in accordance with organisation policies and procedures and the individualised behaviour support plan
b. Focus on the person’s individual needs, strengths, capabilities and preferences
c. Identify problems with engaging or motivating the person
d. Seek appropriate assistance when required
e. Provide a safe environment for the person conducive to positive and adaptive responses
f. Follow organisation procedures to ensure the safety of the person, self and other people
g. Respond to critical incidents in accordance with organisation’s intervention and notification procedures

Case Study 1: Tony Smith
Tony is a 45 years old male who recently had a motor vehicle accident. Tony was healthy and energetic person before the accident. He was a working as a fitness instructor from last 15 years. Tony has a wife (Lora and two boys (Tim and Gayle). Tony was well known for his contribution of mentoring the young kids at footy club.
Tony had lost both his lower limbs post the car accident. Tony returned from the rehabilitation facility and now lives at his family home with Lora and two kids. Tony requires assistance with ADL’s. (activities of daily living)
You are working as disability support worker who provides personal care assistance to tony at his place. One-day Lora (Tony’ wife) spoken to you that Tony does not get involve with family activities. She verbalises the concern that Tony tends to avoid the kids and family gathering. She is afraid that this is effecting Tony’s health.
When you spoke to Tony, He expressed that he is feeling helpless, lonely, isolated and depressed. Tony fears that Lora is going to leave him. Tony is not happy that he is not contributing towards his family and society.

John is an 80 years old male, who has been living in Rosedale facility from last 10 years. John is very calm and quiet person. John required minimal assistance with ADLs. John has always been continent. He likes participating in the social club activities.
Recently John has been diagnosed with dementia. He has been verbally and physically aggressive towards the staff from last few weeks. John has recently been wandering around the corridors of the facility, picking up fights with other co-residents and facility staff.
You have also noticed that John is Incontinent both urine and faeces.
Today his sister has come to visit, and John will not allow her to come in to his room. Attempts by his sister to clarify the situation lead to verbal abuse and incoherent muttering.

Case study 3: Gina
Consider you are working as disability support worker in the Government supported accommodation (Rosedale facility, 442 jukes Rd NSW). You are working on the morning shift. Gina is a 50 years old woman with intellectual disability. She lives in the Rosedale facility with other five co residents. Gina is quite aggressive by nature. You have witnessed critical incident involving Gina and other co resident.
Incident details:
Gina was sitting in the lounge room with Mr Joe Bell. Gina had an argument about T.V program with joe. Gina hit Mr Joe in the head with her walking stick. Mr Joe sustained laceration on the forehead( left side) and he is bleeding. Gina is also verbally aggressive towards the staff. You have separated both residents in the loungeroom. You asked co-worker to take Gina to her room. You have made joe comfortable. And applied clean dressing and stay with Joe until RN div1 arrived at the scene. You have notified the RNdiv1 in charge on the duty about what you witnessed at the scene.
Rn in charge have assessed Mr Joe for other possible injury and applied the wound dressing for the bleeding laceration. Mr joe needed to be reviewed by the GP regarding further management
Mr joe son(Luke) has been notified about the incident.
