Patient Background Details
In context of the given case scenario the patient, James Silva, appears to be suffering from
multiple chronic diseases. He is a 46 years old married man, who have 4 children and lives in
capital city area. In the early age of 24 years he has been diagnosed as been victim of obesity,
which is still continuing in his age of 46 years now. It is evident from the fact that his weight got
increased by 2 kilograms within 2 days (from Wednesday to Friday, where he was last diagnosed
on Wednesday). Also he has Type 2 (non-insulin dependent) diabetes, which has been firstly
diagnosed around 10 years ago. From the time of diagnosis of James is under continuous
observation and cure. Before three years he found himself being victim of Renal Failure, of
which the most obvious reason has been identified, was his diabetes mellitus.
To get treatment of renal failure James is undergoing through regular Haemodialysis. His
dialysis activity is being performed for 3-4 hours a week and almost three times, such as
Monday, Wednesday and Friday mornings. Continuation of Obesity and high blood sugar level
(because of diabetes) has resulted in diabetic foot ulcer, which has affected his left outer ankle
bone. At the time of dialysis the dialysis nurse has dressed it with help of hydrogel. In a
conclusive manner the comprehensive health issue diagnosis details of the James, can be
mentioned as:
 Diabetes mellitus
 Early stage diabetic retinopathy (both eyes)
 Renal failure
 Haemodialysis
 Diabetic neuropathy (both feet)
Identification and Prioritization of Clinical Issues
As per the analysis of the case based scenario it can be easily inferred that initiation of all health
related issues have made from the rising Obesity from the age of 24 years. As a consequence of
rising obesity several other related medical issues came into existence such as insulin resistance,
coronary heart disease risk and adult-onset (Type 2) diabetes mellitus, etc. (DWP, 2008) Similar
is the condition of James, where his sustained obesity has resulted into insulin restriction and
development of Type 2 diabetes mellitus. Advent of Diabetic symptoms in body has been
diagnosed around 10 years ago and its uncontrolled state has further enhanced the medical
complications. High blood sugar level in the body has affected the appropriate functioning of the
other body parts and severely damaged the vision of the patient under consideration. Due to the
uncontrolled diabetic state patient has to bear with Renal failure and undergo with the respective
treatment processes like Early stage diabetic retinopathy and haemodailysis.
In recent routine diagnosis it has been identified that James has developed a diabetic foot
ulcer on his left outer ankle bone, which has been caused because of uncontrolled blood sugar
level, which prevents any cuts, burns and external or internal injuries from healing. In
