    Topic background:


    Tran has attended the community clinic in South Melbourne for support with managing her HIV. Tran is recently diagnosed with HIV and requires a holistic care plan to be developed. This is the information from Tran’s referral.


    Tran is a 30-year female who lives in transitional housing. She has arrived in Australia from Vietnam two years ago and is currently unemployed. Tran has experienced periods of homelessness since she has become estranged from her family after her HIV diagnosis. Tran speaks limited English and has been difficult to engage in consultations

with the treating team at the clinic as she appears dismissive and misses appointments regularly. Tran regularly attends the community drop-in centre for meals and food vouchers and has no other supports in Melbourne. Tran has a history of intravenous substance use and has declined to engage with Drug and Alcohol services. Tran has been prescribed Zidovudine at her last visit to the clinic last week; however, she has not returned for blood tests to monitor her CD4 levels. The GP at the clinic has expressed concern about Tran’ recent weight loss and medication adherence and has requested follow up from the community nurses.

Introduction
Holistic care is defined as a thorough and in-depth understanding of a patient’s complex needs and addressing their social, physical, emotional and psychological state of mind to create a care plan that enables them to deal with their illness and lead a healthy life. During an illness, a patient loses their ability to look after themselves and hence they depend on holistic care to provide them support.
Florence Nightingale was the first nurse who developed the concept of holistic nursing plan that is widely emphasised and used in the healthcare sector today (Haldane et al., 2017). Tran has a history of HIV as well as substance abuse. She has been missing her appointments, which is not a good move because that will progress her disease further and the consequences will be serious.
Hence, it is necessary to make a holistic plan of care for Tran, which will cover her HIV treatment and management plan along with a proper treatment plan for drug abuse. My care of plan includes a wide range of approaches mainly including treatment, education, medication, self-care, nutrition and alternative forms of treatments.
According to Jasemi et al. (2017), holistic nurses are characterised by their professional commitment, social relationship with patients and ethical beliefs. I believe all these principles are extremely necessary for a healthy care plan to be established and building communication skills with the patient is the key to attain it.
Part A:
1. Significance of Holistic Care for Supporting Tran
As Tran has been known to have HIV and she is not taking her treatment diligently, hence I have decided to develop a care plan for her. People having HIV or AIDS are prone to severe infections because of compromised immunity hence a systematic approach needs to be established. Due to recurrent infections, there is a repeated need of hospitalisation for such patients over the course of the disease and it increases their chances of mortality as well as morbidity especially when a patient develops resistance to antibiotics.
According to Zepeda et al. (2019), Highly Active Antiretroviral Therapy (HAART) is known to decrease the progress of HIV in people but it is the non-adherence to medication, which is known to cause complications of the disease and developing opportunistic infections. Since Tran has no proper accommodation and is homeless, hence, I have decided to extend palliative care so that she can continue getting proper treatment. She has no access to proper food and is even addicted to drugs.
Tran has supposedly acquired HIV due to sharing of needles with multiple people because intravenous injections provides instant bioavailability and the drug gets instantly circulated into the system thereby reaching effecting brain. According to Wang and Maher (2019), drug addiction is known to cause psychotic disorders among patients such as suicidal thoughts, aggression, violence, mood disorders and a plethora of developing other serious illnesses.
Tran has been known to miss appointments with the community clinic treating HIV and even not seeking treatment from drug and alcohol services, which is a very common pattern, observed amongst illicit drug users due to societal stigma and fear of judgemental attitudes from healthcare staff members. According to Wieman et al. (2018), stigma associated with HIV and Drug use is the main factor responsible for patients not turning up for treatments because they are made to feel undervalued and their social service needs are not met by the practitioners.
