You are to complete a community health needs assessment (CHNA) for Hillsboro County utilizing the information contained in Case 1 of the Middleboro CASEBOOK. A CHNA answers the question: What are the primary healthcare needs and issues facing this county?
You are encouraged to seek outside data to benchmark local data to state and national data/trends. For purposes of benchmarks, assume that Hillsboro County is located in Pennsylvania. Selected Excel tables of data from the case are provide in Moodle.
Your submission is limited to 5 single-spaced pages following APA format. Utilize appendices for tables and charts (not included in page count).

Demographic and Population Analysis


We need an analysis of the population changes and characteristics in Hillsboro County. Please address each of the following questions and provide appropriate quantitative justification to support your conclusions and recommendations:


  1. Over the past five and ten years, which of the communities located in Hillsboro County have grown faster than the overall rate of population increase in Hillsboro County? Over the last five and ten years, which of the communities in Hillsboro County have grown slower than the overall rate of population growth in Hillsboro County?

  2. What should we expect the population to be as of CY+5 for all towns located in Hillsboro County?

  3. To what degree does the age profile of Hillsboro County resemble the overall age profile demographic of the United States? Which towns in the county are the “oldest” and the “youngest?”

  4. Assess the implications of your findings related to the delivery of health services in the county.




Nursing Home Need Study


National statistics indicate that approximately 3.9 percent of the population 65 or older is in nursing homes. Historically, nursing homes in Hillsboro County have an averageoccupancy of 94 percent. Does Hillsboro County currently have a sufficient number of nursing home beds? Are additional nursing home beds needed as of today and/or by 2020? If yes, indicate where in Hillsboro County you recommend any additional nursing home beds be located.

Health Priorities


In order to assess and improve health services in Hillsboro County we need a comprehensive understanding of population characteristics and anticipated changes. For example, the need and demand for health and medical care are both critically linked to demographic characteristics. Based on your answers to Question 1, indicate your five most significant assessments involving Hillsboro County and its communities. For each critical assessment, indicate your rationale and assess its significance on the health and healthcare services in Hillsboro County.

Hospital Comparison


We need an analysis of hospital utilization, cost, and quality in Hillsboro County. Consider average length of stay (ALOS), hospital admissions and patient days per 1,000 population, and other appropriate statistics. Answer the following questions and provide appropriate quantitative justification to support your conclusions and recommendations:

County-Wide Disaster Planning


As you are aware, under a new federal and state program administered by the office of the governor, every county has the opportunity to apply for a planning grant (potentially), followed by implementation funding to better prepare for large-scale disasters. Each municipality in our county and all licensed health facilities and schools has existing disaster plans. Some of our larger employers also report have such plans. An analysis two years ago indicated that the number of first responders (police, fire, and EMT) in Hillsboro County per 1,000 residents is within 5 percent of national averages for a rural area. As such, we believe that we have sufficient infrastructure to address accidents and related issues.
For the grant we need to propose six to ten actions that could significantly enhance our area’s ability to address large-scale weather disasters and/or other types of large-scale disasters that would involve a significant area response. Provide your list of six to ten actions with rationale.

Answer 1):
Table 1 of the case clearly provides an indication about the growth in the various
communities that are located in Hillsboro over the period. The five year growth rates are
determined using the 2014 and 2009 data and 10 year growth is determined using 2014 and 2004
data using the below formula:
5-year growth = (2014 Population – 2009 P
opulation)/ 2009 Population
10-year growth = (2014 Population – 2009 Population)/ 2009 Population
From the analysis, it is clear that over the five-year period Statesville population has
grown faster at 12.55% and over the ten year period Statesville population has grown faster at
21.71%. Jasper and Statesville population growth was higher than the country population
growth over the past five and ten years. Middleboro had the slowest growth in five years at
0.48%, and in ten years their population increased by 1.27%.
Over the five years Middleboro, Mifflenville, Minortown and Boalsburg had the slower
growth than the population growth of country and Harris City, and Carterville posted negative
growth in population over past five years. Over the ten years Middleboro, Mifflenville,
Minortown and Boalsburg had the slower growth than the population growth of country and

Answer 2):
Various factors have impact on the future population of the Hillsboro County. The factors
include, increase migration from another part of the country, child birth rate, the death rate of
people, the percentage of people with disease and percentage of people who are healthy, etc. In
this case, the historical growth rate in the population is taken into consideration as the historical
growth rate in population includes all these factors. In this case, five-year growth rate between
2004 and 2009 and 2009 and 2014 is determined using the below formula.
5-year growth (2009-2014) = (2014 Population – 2009 Population)/ 2009 Population
5-year growth (2004-2009) = (2009 Population – 2004 Population)/ 2004 Population
Percentage change in 5 year growth rate = (5-year growth (2009-2014)) - 5 year growth (2004-
2009))/ 5-year growth (2004-2009).
Percentage change in 5-year growth rate is determined to obtain an outline about the
actual change happened in the population growth rate. Then the future growth rate is projected as
2009-2014 growth multiplied by (1+ Percentage change in 5-year growth rate) for five years.
Middleboro 0.29%, Jasper 2.51%, Harris City (-8.53%), Statesville 19.34%, Mifflenville
108.12%, Carterville 15.85%, Minortown (-0.85%), Boalsburg (-51.24%) and overall 2.16%
growth in population. (Refer Appendix 2).

Answer 3):
The resemblance of overall age profile demography is similar (Infoplease, n.d.) (Refer
Appendix 3). Both has the highest population for the age profile between 25 and 44, followed by
45 and 64 and it is at least for 75 +. The demographic distribution based on the age profile of
Hillsboro County depicts the US demography based on age profile. The oldest town in Hillsboro
County is Middleboro that has 4,420 people above the age of 75. The youngest town of Hillsboro
County is Jasper with 4,128 children under the age of 5.
Answer 4):
Both the oldest and the youngest among the population are the two major segments who
always require medical attention and services. The county has to focus on establishing more
health services in Jasper and Middleboro since Jasper has more children and Middleboro has
older adults. Similarly, Jasper and Middleboro are the highly populated region of the county that
requires more health services. Elderly people need geriatric care as they need both psychological
and physical care. The county will require allocating more funds for the elderly care in
Middleboro and childcare in Jasper. There will be more demand for the health-care services in
the county as there will be an increase in the aging population. In Hillsboro County, the
population growth is decreasing indicating about the future growth in aging population.
Therefore, there will be demand for more elderly healthcare services in the future.
Nursing Home Need Study
From the statistics, it is clear that about 3.9 percent growths in population over the age of
65 years and above in Hillsboro County. There are a sufficient number of beds is available in
Hillsboro County. But the level of increase in the aging population is lesser than the expected
increase in the overall population. It indicates that the birth rates are lower in the county and by
2020 there will be a significant increase in the people above the age of 65, and therefore, there is
be a requirement for more home beds in the County. The County has to increase the nursing
home in Middleboro that has older population.
Health Priorities
Middleboro and Jasper are the highly populated region of the County, but the growth in
