For this assignment, you are tasked with creating a short, self-administered survey. Your survey should be clear and understandable such that someone could pick up the survey and take it entirely on their own (e.g. without your help to guide them through the questions). Your survey should be 2-4 pages in length and have about 25 questions. Before you start, read Chapter 8 in your textbook for guidelines about survey research and writing good surveys.



Survey Goals:Imagine you are designing a support program in San Diego for families that have a loved one who is incarcerated. You realize that you need to better understand what these families need and want in order to inform your program design. You decide to develop a survey to help you gather this information. The survey will be sent out to hundreds of families in San Diego that have a loved one who is in jail or prison. The survey should ask about their experiences, concerns, needs, and assess their interest in joining a support program like the one you are developing. The survey should be thoughtful, sensitive and clear.


Your survey should include:



  • An opening description of what the purpose of the survey is, why the respondent is being asked to take it, any benefit to the respondent for taking it, instructions they may need for how to take the survey (including how to return the survey), and an explanation as to the extent of confidentiality and anonymity that can be expected, etc.

  • Clear instructions for each section of the survey about how the respondent should answer the questions (e.g. “select the answer choice that best represents how you feel” or “rank the following from 1 to 10 where 1 represents your smallest need and 10 represents your biggest need” etc.)

  • Well-organized and clear section headings and clearly numbered questions to help guide the reader through the survey.

  • Demographic questions (e.g. sex, age, race/ethnicity, level of education, religious affiliation and/or other background questions about your participants that you think arerelevant to the purpose of your survey). Remember that we don’t want to collect personal information about people that is not necessary for us to achieve our research goals. Be thoughtful about what demographic information is important to collect.

  • A combination of close-endedandopen-ended questions to address your survey goals. Your survey should have a good mix of both types of questions.

Hello, we are designing a support program for families whose dear ones are currently serving a term in
prison. This initiative has been conceived to lend the grieving families a helping hand and a resting shoulder. We understand that it may have been a heart wrenching experience. Some of you must have felt stressed and lonely. To this end, we have planned this program and we require your little help. You need to kindly fill this small survey questionnaire.
