Drugs may be used in mild disease and while awaiting a transurethral resection of the prostate. Selective α1-adrenoceptor antagonists increase urine flow to a limited extent but also decrease urgency, frequency and hesitancy. The 5α-reductase inhibitor finasteride slowly reduces prostate size.


1. α1-Adrenoceptor antagonists can cause postural hypotension, especially with the first dose. They cause dizziness and can interact with other drugs to lower blood pressure. Finasteride can reduce libido and cause impotence.


2. The outcome is variable; symptoms may not worsen appreciably for many years, but moderate symptoms can lead to a poor quality of life. Complications include urinary retention, incontinence and renal insufficiency owing to hydronephrosis.
