What You Need to Do:


CASE STUDY – Johanna Smith: Johanna Smith is an 84 year old woman who lives at Happy Valley Aged Care Facility in Brisbane. You, a QUT nursing student, just started your morning shift at 06:30 and when you enter her room you find Ms. Smith on the ground as is shown in Picture 1. She has experienced an unwitnessed fall. Picture 1: Ms Smith has a medical history of osteoarthritis and Type II diabetes. She uses metformin 500 mg three times daily for her diabetes and she is on regular analgesia. She uses paracetamol 1000 mg four times daily and ibuprofen 200 mg prn (if required) to manage her joint pain. She has had two other fall events in the last six months and uses a walking stick when mobilising as she has stated that she ‘feels wobbly’ when she walks.


After reading the case study respond to questions 1 and 2.


Question 1 (500 words):


A.Describe and justify, what two (2) nursing assessments should be performed as a priority in this case.


B. Identify two (2) factors (physical or environmental) that could have contributed to Ms Smith’s fall and explain these factors in relation to underlying pathophysiology.


C. Using the SMART Goal framework, identify a priority goal for care. D. Using current and relevant evidence, identify and justify two (2) evidence based nursing interventions to reduce her risk of falls in the future. Support your response with relevant and current evidence.


Question 2 (250 words): Based on Ms Smith’s recent fall event document a written entry into the progress notes.


Your report must use the SBAR framework and accurately report the fall incident (Situation), identified risk factors for falling (Background), assessments and interventions suggested in Q1 (Assessment), and how the interventions should be evaluated (Recommendations).


Length: 750 words (+/- 10%) Word length includes in-text referencing and excludes your reference list.


This assessment task must respond to the question in the ‘Task Description’ and ‘What You Need to Do’ sections.




Question 1 must be written using formal academic sentence structure (i.e. complete paragraphs that are grammatically correct and with in-text referencing). You can use textbooks as a reference; however, you are also required to use at least two (2) relevant and contemporary peer-reviewed research articles. The videos ‘Getting Started with CINAHL’ and ‘How to Locate Your Readings’ in BB will show you how to find these research articles. The use of the assessment question as headings is acceptable, but not mandatory. Due to the small word limit, an introduction and conclusion paragraph is not required.




Question 2 you must use the SBAR framework and full sentences that are grammatically correct.

Question 1:
A. The case study includes the analysis of the condition of an 84-year-old patient suffering from osteoarthritis and diabetes-II problem. The nursing assessment for this patient will be X-ray imaging of the bones which are degenerating and getting degraded with a physical assessment of the bones to analyse the amount of swelling, redness and inflammation in the joints (Belleza, 2021). This patient is prescribed metformin for diabetes-II, paracetamol and ibuprofen for pain and swelling in the joints. 
B. The two factors which are responsible for fall injury in the patient are the degeneration, and degradation of the bones leading to the bones turning weak and fragile as old age advances. Further this patient has diabetes-II due to which the chance of falling in the room increases due to loss of balance and locomotion coordination. 
C. The necessary nursing interventions will be to undergo a loss of weight therapy so that lesser fat deposits in the body. This patient should undergo a regular exercise regime and she should take pain killer medicines regularly as prescribed by a doctor and nurse. These nursing interventions will reduce the chance of falls and injury in the future (Belleza, 2021).
