DIRECTIONS The Med-SLA provides you with the opportunity to demonstrate your clinical development in the key areas of the Australian Commission. The Med-SLA can be in one of the following formats: A product An intervention (implemented or proposed) An activity (implemented or proposed) Possible areas that you may wish to consider for your Med-SLA (remember you only need one) medication reconciliation medication information resource Environmental safety scan In-service Short CPD course eg. NPS Medicinewise course (Free, evidence-based and online) WHAT DO YOU NEED TO SUBMIT? A brief (150 word max) dot point outline of your proposed Med-SLA under the following subheadings (note this forms the basis for the final report): Your employing agency’s name, the area you work in, the primary clinical focus The focus of your Med-SLA project The product/ intervention/ activity you are proposing Justification for what you are proposing (why does it need to be done and evidence base) The potential outcomes of the SLA in your agency A sample of the final Activity/Product/ policy you created or participated in Eg. A poster as shown in example 250 word A 1250 word Med-SLA report addressing the following subheadings: Your employing agency’s name, the area you work in, the primary clinical focus The purpose and aim/s of your Med-SLA project and how they support the Australian Commission of Quality and Safety in Health Care guidelines and safe nursing practice The changes to your practice as a registered nurse that you have made or will make in response to your Med-SLA. The process used to complete the Med-SLA The potential outcomes of the Med-SLA in your agency including how your Med-SLA could be accessed, improved and evaluated in future.
