Details:The Professional Capstone and Practicum course stands apart from prior programmatic courses in that each student will select an informal course mentor in his or her current place of employment. If the student is not currently employed or if the student elects, this may take also place in a volunteer health care setting. This mentor is not a preceptor, but an individual who will guide the student with organizational navigation as he or she works to complete the practice immersion hour requirements for this course. Students are not required to “shadow” their mentor.Selection, approval, and retention of a Course Mentor must be completed as soon as possible at the start of this course, as this individual will be working with the student on each assignment that incorporates practice immersion hours. Working with a mentor is mandatory. Your mentor may not be your faculty member.Students will select an individual at their selected health care setting to serve in the informal role of Course Mentor. This individual will work closely with the learner on the entire scope of Capstone Project-based deliverables throughout the course. Note: the Course Mentor is a professional resource for the student and will not be involved in grading course deliverables; only the course instructor will serve in that role.The Course Mentor should be someone who can provide the student with solid knowledge and candid feedback, assistance in organizational navigation to enable the student to achieve practice immersion hour requirements, and who possesses a genuine interest in working with the student to further his or her academic goals.After students have identified a prospective Course Mentor, they should obtain approval from the course instructor immediately to ensure assignments with practice immersion hours requirements can proceed in a timely manner.In order to avoid delay in commencing work on your course deliverables, please identify your Course Mentor as early as possible in this course.Once a mentor has agreed to serve in the role, you must complete and submit the Field Experience Site Information Form and the Qualified Mentor/Preceptor Summary Form to your faculty for approval in the LoudCloud classroom Individual Forum immediately so that practice experiences can begin in a timely manner. Delays in identifying and completing the appropriate paperwork may result in difficulties in successfully completing the course. The student is responsible for obtaining all signatures and paperwork submission. Course faculty will approve all sites and mentors according to student learning needs and specific course objectives. A Memorandum of Understanding (MOU) for the approved mentor must be on file in the LMS classroom. Prior to the start of the field experience, a copy of the mentorâs license and certification, and an abbreviated CV must also be on file in the LMS classroom for faculty approval. Agencies sometimes require an Affiliation Agreement, if that is the case please contact the OFE as soon as you are aware. If required, please contact the Office of Field Experience (OFE) at 602-639-8411; office hours are 7:30 am â5:00 pm Monday through Friday.Do not initiate new work on practice immersion hours-based course assignments until your course instructor has approved your Course Mentor.Once a mentor is approved, the student should contact him/her within one week of receiving the approval notification.
