By January 15, to develop a sense of community among residents of Farmington as measured by their meeting together on a regular basis. By one year after the youth enters the support program, 90 percent of the youth will have his or her G.E.D. or will be enrolled in and regularly attending a local high school. This will be documented by his or her caseworker in the form of G.E.D. test scores, registration forms, attendance records, and continual contact with the school. By the end of their first year in the program, 70 percent of stroke survivors will have maintained or enhanced their levels of rehabilitation as measured by an increase of at least 50 percent in their pre- and posttest scores on the Barthel Index, which measures activities of daily living and mobility and by pre- and posttest scores on the PULSES profile, which provides a global assessment of functional status. These assessment instruments will be administered by the social work coordinator of this program. By January, all home care recipients over the age of 85 will be assessed by the social work and nursing case management team, using the multidimensional OARS assessment tool developed by Duke University.
