Every year more than 40 million people worldwide need palliative care due to several chronic diseases like cancer, lung diseases, liver or kidney failure, Alzheimer’s disease, cardiovascular or neurodegenerative diseases, which also involves 2.1 million children. However, only 14% of the people presently receive it (World Health Organisation, 2015). The major aim of palliative care is to improve the quality of life in terminally ill patients. It requires effective psychological care and symptoms management. The report presents a comparative study of the issues associated with the patients receiving palliative care at home based and in-patient settings.
A research by Behm (2015) reviewed that the home based palliative care has fewer rates of hospitalizations, and fewer emergency department visits. They are cost effective and the patient reports to have better quality of life and satisfaction levels. However, most of the health insurance providers do not cover home based palliative care, so it increases the financial burden on the patients. Sometimes these patients do not readily get access to intended physician visit at home.
A retrospective study by Peters (2006), found that the patients receiving home based care had considerably low depression, distress and severity than those receiving in patient care. The home based patients had better health, improved quality of life and more control over their symptoms than the in patients. However a qualitative study by Danielsen (2018) identified that the home based terminally ill patients perceive more death rates and shorter life expectancy. The main causes attributed to these issues are lack of close collaboration between the family, patient, general practitioners and the home care nurses.
