You are to submit a client intake form that you have devised for your firm to use.It will be up to each of you to develop a client intake form that includes the important information that you expect to glean from an individual ( not a business) client in your first interview with them. If your firm is going to be focusing on a particular area of law do not make assumptions as to who your client will be, for instance if your firm is going to focus on civil litigation do not make assumption on your form that you are representing a plaintiff/defendant or an applicant/respondent Your client intake form should reflect either possibility.This form must be word processed and formatted professionally. It should be an absolute maximum of 3 pages.You will be acting as a sole proprietor and your letterhead needs to indicate your name as that is how I will identify you for grading purposes. Do not include a cover page.
