Choose one of the two following assignments and write a brief summary of a few paragraphs; no longer than one page. Please note: That this assignment concerns case management (not managed care). Refer to case management section on page 249 and outpatient long-term care services section on page 168 of textbook. ( look at the attach file!!!) Have you seen real-world examples of case management services in action (at your workplace, in the life of a loved one, etc.)? If so, tell about the experience. Was it effective, helpful, etc.? Why or why not? What tasks did the case manager perform? Locate an agency or institution in your community that provides case management services. It may be a rehabilitation hospital, state department for aging, community mental health clinic, training center for the disabled, retirement community, etc. Name the agency or organization, provide its location and its type (what kind of facility is it?). Tell what services their case managers provide. You may find this information by searching the facility's website, dropping by to pick up one of their brochures, interviewing a patient / client whom you know has received their case management services, etc.
