C.B. is an 88-year-old male client with impaired cough reflex and swallowing mechanism. He has just returned from surgery with a decreased level of consciousness.

1. maintain the head of the bed elevated at 15 degrees.
2. have suction available at all times.
3. contact speech pathology to evaluate the client’s swallowing ability.
4. provide thin liquids to drink when the client fully awakens.
