Aim of Assessment
The purpose of this assessment is threefold. For the student to:
1. Identify, using a visual format, the processes involved in health breakdown affecting alterations in any of the following areas, eating, drinking, nutrition, elimination and sexuality.
2. Briefly discuss pharmacological concepts and their safe, effective application in professional nursing care related to alterations in any of the following areas, eating, drinking, nutrition, elimination and sexuality.
3. Identify the use of specific diagnostic procedures that may be used to confirm specific health breakdown diagnoses relating to alterations in any of the following areas, eating, drinking, nutrition, elimination and sexuality.

Concept Map:

- The concept map must be computer generated.
-When researching the topic, use the template, as described in Lecture 1, to assist you in organizing your information. The template will also assist in identifying key concepts and their relationship/s.
-The concept map is to be illustrated on one A4 page. Use APA convention to reference citations in the concept map and include these in the reference list.
-Concept map development clearly, yet concisely summarizes the most important information identified into what is known as ‘key concepts’. It also conveys the relationships or links between these key concepts. Critical thinking and creativity are essential components of this assessment. Concept map design needs to be visually stimulating.

- You are to answer two (2) questions related to the topic.
- You are expected to paraphrase the literature and reference all work from others in accordance with academic standards.

  • In-text referencing ideas are referenced correctly throughout the essay including appropriate acknowledgement (referencing) of all the words of others. Reference list to include at least five current, relevant academic journal articles.

Introduction:
Structural and functional changes that occur in the kidneys as Chronic Renal Failure develops:
Chronic kidney disease (CKD) is common in all age groups, including children. CKD is caused
by man
y reasons and is characterized by progressive scarring that ultimately affects all structures
of the kidney and cause chronic renal failure.
The structural changes that associated with chronic renal failure:
Several structural changes can be observed in renal failure. Glomerulonephritis is one main cause
of change of kidney epithelium. It is caused by inflammation of the kidney and it damages the
portion of the kidney that filters blood, i.e. the glomerulus. Urinary tract obstruction can occur at
any point in the urinary tract, from the kidneys to the urethral. Calculi, tumors, strictures, and
anatomical abnormalities etc can cause urinary obstruction. Obstructive neuropathy can result in
pain, infection of tract, loss in renal function, or ultimately renal failure. Chronic renal failure
may result from renal artery stenosis in which the artery that supplies blood to the kidney
becomes narrow. Narrowing of renal artery usually occurs in atherosclerosis patients where
plaques of cholesterol and fat accumulate at the wall of the artery. Renal artery stenosis prevents
the blood flow to the kidney, and gradually decreases the filtration function of kidney which
ultimately causes renal failure. Chronic high blood pressure damages the kidney tubule and
glomerular artery and cause kidney dysfunction which is causes Hypertensive nephrosclerosis.
Chronic renal failure is also associated with renal vein thrombosis in which one or both renal
veins become blocked. It can occur by several causes such as: Abdominal aortic aneurysm,
Clotting disorders, Dehydration, nephritic syndrome, trauma etc. this disease causes decreased
urine output, back pain, bloody urine. Focal segmental glomerulosclerosis (FSGS) also occurs in
renal failure. It is a disease in which scar tissue develops on the parts of the kidneys that filter
waste out of the blood (the glomeruli). FSGS can be caused by a variety of conditions. Without
treatment, primary FSGS will usually lead to kidney failure.
