CASE STUDY ONE
You are a RN working in ED, looking after Mrs Wong, an 93 -year-old with an admitting
diagnosis of Acopia. Mrs Wong is alert and orientated, has mild urge urinary
incontinence and general frailty. All acute interventions have been completed for Mrs
Wong, but the wards are full, so she will spend the next 8 hours (+) on an ED trolley in
a corridor.

1. What is the potential impact on staff and patients of a diagnosis based on
labelling of a patient? In this case the label of ‘Acopia’.

2. How do normal physiological changes of ageing increase the risk of Mrs Wong
developing a pressure injury in this scenario? Consider integumentary system
plus 2 other body systems in your answer and provide detailed science based
rationales for why physiological changes mentioned impact pressure injury risk
for Mrs Wong.

3. Provide detailed evidence based nursing interventions with rationales that
should be implemented for Mrs Wong to reduce the risk of her developing a
pressure injury while in ED? Do not include referrals in your answer.

CASE STUDY TWO

You are a RN working in community care, visiting Mr Adomos, a 78-year-old Greek
gentleman with a chronic and painful venous ulcer on his lower leg. He takes multiple
medications for chronic illness, and a codeine-based medication for pain. Mr Adomos
lives alone in his own home.

4. How would Mr Adomos’ chronic pain impact on his ability to maintain a safe
environment?

5. Why is Mr Adomos, as an older adult, more vulnerable to adverse drug events?
Consider anatomical, physiological and behavioural considerations associated
with ageing.

6. Detail evidence based nursing interventions and rationales to reduce the risk of
Mr Adomos’ experiencing an adverse drug event in relation to his codeinebased medication. Do not include referrals in your answer.

Case Study analysis of Mrs. Wong
1. Potential impact on staff and patient with label Acopia
Mrs. Wong is 93-year-old women who has admitted to the hospital to the mild urge urinary incontinence and general frailty. Acopia is the term used to describe the inabilities of the physician to treat older patients who are unable to cope with their daily ac
tivities (Noian et.al., 2018). It is most commonly observed among the older patients with the complex medical and social history who lost functional abilities w.r.t to functional mobility and who needs immediate concern of treatment with doctors. From the literature it was understood that most of the patients who got admitted with Acopia have symptoms of geriatric syndrome, falls, confusion, morbidities and incontinence (Curran & Chattopadhyay, 2015). Most of the patients who are admitting to the emergency departments are casual and unhelpful. Curran & Chattopadhyay (2015) has conducted the studies to understand the behaviour of the 146 respondents with different specializations who have encountered the Acopia labelled patients. Some doctors support that labelling the patients has reduced awareness in providing the reversible treatment for the elders. Most of the labelled patients use inappropriate, unprofessional and singular unhelpful patient care for those who are having the long-term frailty and cognitive impairments (Oliver, 2008). The studies of the Hodges et.al. (2021) has concluded that it is very important to use proper language and most of the practioners lacks the awareness to treat the patients (Hodges et.al., 2021). This improper knowledge of the patients will result in more complexities and may increase the mortality rates pf the admitted elders due to disease complexity. Hence, it was understood that most of the healthcare workers lack the knowledge about giving the proper care to the old people. It was very important to provide a specific awareness and training to treat the older people with Acopia.
2. Physiological changes of ageing and associated impact pressure injury risk w.r.t body system
The most commonly reported changes w.r.t to increased age are gastric disorders such as pressure ulcers, loss of mobility due to improper integumentary working and memory loss of the patients due to degenerations of some of the motor neurons. The studies of Jaul (2010) has concluded that the pressure ulcer is a chronic disorder by which most of the elder patients are affected (Jaul, 2010). Some older patients report grade 3 & 4 ulcers which turns into complex wounds and untreated or improper care of the wounds may result in the death of the patient. The pressure ulcer is associated with the geriatric syndrome that may have the impact on the mobility, nutritional deficiencies, poor immune system activity which increases the elder person more vulnerable to other diseases.
There will be a slow and steady physical impairments and functional disabilities with increase in age and is a biological mechanism which can’t be controlled (Amarya et.al., 2018). With increase in age of the people most of the people lose calcium levels and due to improper nutrition balance the elder persons will encounter the balance problems such as falls (Heaney et.al., 1982). This can be prevented by educating the older people with the use of some support equipment to manage their daily activities.
Memory loss is the most promise complaint among the elder people and this is due to the degeneration of the motor neurons and improper...
